Navigating Pacific Wellness Service
The Navigating Pacific Wellness Service aims to support Pacific families to live healthily and independently. Our Pacific health navigators to work with Pacific people in the community to help navigate families to be confident and resourceful in their health and well-being.
The services empowers families to make decisions about their health, and supports families to live in hygienic, warm, uncrowded and safe homes. Pacific parents are supported to give their children and young people the best possible start in life and Pacific families are helped to self-manage health and make decisions about their own wellbeing.
The navigation service connects with other services, including:
- Best start to life – antenatal/postnatal care and 0-4 years
- Life launch 5-15 years
- Youth resilience 15-24 years and families
- Healthy adults 26-64 years
- Rheumatic fever education – all ages
- Pasifika Ola Lelei – all ages
Criteria / Cost
The FREE Navigating Pacific Wellness Service is available to Pacific families living in the Counties Manukau area, identified by South Seas teams who may benefit from Navigation Services.
